Submit a completed Application for Service form with the appropriate deposit.

• Small Business – $100
• Restaurant – $150
• Greenhouse, Carwash or Laundromat – $200

In addition you may also be required to abide by our Pretreatment Program if applicable to your business. Please contact our  Pretreatment Coordinator –  John Hale     Phone 859-336-5454 for more information.

If a new or resized meter is required please also submit an Application for New Water Meter Service.
